Welcome to the Office form, fill it out, and bring it with you for your appointment. This will help us help you more quickly.
For Patients With Insurance:
The world of insurance coverage is complicated. To figure out your benefits and make your visit go faster please help us by doing the following:

Bring your Medical Insurance card and your Vision Insurance card, if available, if you will be using insurance for your visit. We need this because sometimes eye care is covered by your medical plan and sometimes it is covered on the vision plan |

Find out if you have a PPO or HMO. If it is an HMO, find out the name of the group and your primary doctor. If you have a HMO, you may need a authorization to see us. If you are not sure, call and we can check for you. |

Remember that co-pays are due the day of the visit. Also know that we can help you with insurance but we, as providers, do not guarantee any payment or validity of any information we may receive from your insurance company. |
